What this site stores today
The consultation form stores your name, email, phone, age group, preferred centre, the pillar you are interested in, the goal you describe, your preferred contact time, your IP address and the date you submitted it. Your password is stored only as a one-way hash. Contact messages and newsletter sign-ups are stored until you ask us to delete them.
Sections your counsel must complete
- Categories of personal and health-adjacent data collected, and the lawful basis for each
- How consultation data is used, who inside the clinic can see it, and how long it is retained
- Sharing with third parties — diagnostics labs, payment processors, communication tools
- Patient rights under the DPDP Act: access, correction, erasure and grievance redressal
- Cookie policy and any analytics or advertising pixels you deploy
- Named grievance officer with contact details
- Medical record retention obligations under applicable clinical establishment rules
